The graphic should be linked to a URL such as a "universal" link or an iOS (iPhone, iPod touch or iPad) link.

The "universal link" will work on all smartphones using the built-in mobile browser.  Javascript enabled browsers work best.  For example, on an iOS device such as the iPhone, iPod touch the "universal" link will run your survey using Safari.  On other mobile devices, such as Android and BlackBerry, the survey will use the devices built-in browser, and on the web the survey will be displayed in your default browser, such as Internet Explorer, FireFox, Safari or Chrome.

This link will run the survey in the previously loaded iOS SURVEY ON THE SPOT app, or to run the survey on the browser on any mobile phone, including iOS, Android and Blackberry.
